Embedding form code into a lightbox

i working on simple test page trying figure out lightboxes.

 

i have functional based on examples found online, however, stuck.

 

i can't seem figure out how embed form code lightbox successfully.

 

basically, need replace image pops form.

 

my form code is:

you want run remote called form javascript, , place inside lightbox, that's called jquery, in placeof image... correct?

 

with basic lightbox, that's not idea.

 

you might able better fancybox , iframe plain html page script in it.
